Two cleanings per year. One set of X-rays every 12 months. Exams. Fluoride for kids under 18. Most plans cover these at zero cost if you stay in-network.

Crowns. Bridges. Dentures. Root canals on molars. These procedures split cost evenly between you and insurance.
A crown runs $1,200 at our practice. Insurance covers $600. You pay $600. If you hit your annual max, you pay everything out of pocket.
What's Never Covered
Cosmetic work. Teeth whitening. Veneers purely for appearance. Implants on some plans. Read your policy carefully.
Some insurers now cover implants at 50% if the tooth was lost due to injury or decay. Check with your HR department.
